Staffing Changes

In order to accomodate staffing changes (new account creation, changing or deleting an existing account) the departmental Authorized Account Requestor must submit the School of Medicine Account Request Form online:

https://ucsf.service-now.com/ess/

  • Login
  • Go to "Order Specific IT Services"
  • Select "Accounts and Email"
  • Select "School of Medicine Account Request Form"
  • Complete the form
  • Submit

The Requestor will receive an email confirmation with a Request number.

All requests to ISU for access to employee's electronic records such as e-mail or files must be accompanied by the UCSF Tracking Form for Access With or Without Consent to Electronic Communications Records. Please review the Electronic Records Access Control Policy before submitting form:
