Patient Safety Bulletin

 

UCSF GME is collaborating with the UCSF Medical Center Patient Safety
Committee to publish a monthly Bulletin that describes patient safety initiatives
underway here at UCSF. As GME enters the ACGME’s Next Accreditation System
(NAS) there is new emphasis on engagement of teaching faculty, residents and
clinical fellows in patient safety and quality improvement. We have a robust
quality improvement process in place at UCSF, and this Bulletin is intended to enhance communication with faculty, residents and clinical fellows for education
and engagement.


Each issue of the Bulletin will describe an actual clinical event that triggered an
Incident Report which led to a Root Cause Analysis from which arose policies and
processes to prevent or mitigate similar future events. In addition to the actual
case and the subsequent changes made, each issue of the Bulletin will describe
the purpose of an Incident Report and how to create one. The objective is to
encourage faculty, residents and clinical fellows to identify and report situations
that could result in a bad outcome so that problems can be addressed with broad,
interdisciplinary improvements.
